Transitional Care Management is owned by Dr. Reynolds. TCM provides substance use, mental health, and MAT (Medication Assisted Treatment) services to individuals recently released from Federal prison. Services are provided in Dallas, Fort Worth, Tyler, and Austin, Texas. It is an honor to provide these services and help clients and their families with their transition into the community.

 

TCM also provides mental health, substance use, and psychiatric services to individuals on Federal probation in Tyler and Austin, Texas.

We have wonderful therapists, psychiatrists, and administrative staff that assist the clients and communicate with the residential re-entry centers and pretrial/probation officers to make the transition as easy as possible.
